Responsibilities

Provide direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.

• Implement and monitor patient care plans. Monitor, record, and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.

Requirements

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Additional Preferences

• 2 years of hospital or home health experience preferred.
• Requires holiday and on call rotation 4 - 6 month times a month (5PM-8AM - urgent issues).
• Live within 30-40 minutes
• Sharp assessment skills, critical thinking, organization skills, excellent communication skills and some computer knowledge skills.
• This position requires 6 weeks of orientation with a preceptor.
• Service Area: this position includes but not limited to: Hoffman Estates, Streamwood, Bensenville, Wood Dale, Addison and Elk Grove Village.
• This position allows for some flexibility with the exception maintaining 40 credits per week. The position is salary plus model, any credits over 40 per week will be paid times base rate.
• Documentation is encouraged using point care, which is documenting in the home as much as possible. In some cases, documentation will need to be completed after the visit. Calling patients the night before is an expectation.
